<p>Of course, decisions are always made by the wider group, so let's bring this to the next meeting (there is a lot on the agenda so it would be worthwhile arranging this now). <p style="font-weight: 400">This draft is shared for structured review and feedback at this stage. The aim is to formalize ISCET’s membership and governance framework in a clear, transparent, and sustainable manner as the society continues to develop.</p>

<div>It has been 18 months since the last general update, and as we end 2025 with clinical guideline development now underway, this seems a good time to update on ISCET’s progress. The Society was formed to establish consensus on standardised clinical eye tracking protocols, to provide a unified voice for clinicians using the technology, and to enable data sharing to build normative/reference datasets. All of this should make it easier for new clinicians to enter the field and ultimately benefit patients. ISCET remains open to anyone with an interest in clinical uses of eye trackers, with all decisions being made through participation in our <a href="https://www.jiscmail.ac.uk/cgi-bin/webadmin?A0=ISCET" data-outlook-id="4e237a0b-c407-4783-8ee6-762a5696ff6f">mailing list</a>, <a href="https://clinicaleyetracking.org/community/" data-outlook-id="c6a014b9-7541-441a-9083-3df50724056c">forum</a> and <a href="https://clinicaleyetracking.org/meetings/" data-outlook-id="d8225b7c-d2da-4938-bb50-26e96f075870">meetings</a>.</div>

<div>In collaboration with <a href="https://iscev.wildapricot.org/" data-outlook-id="8825cf1d-eb30-47c9-9e73-f66301366318">ISCEV</a>, ISCET is currently focused on defining protocols involved in the assessment of nystagmus. A dedicated working group has been established, with <a href="https://clinicaleyetracking.org/people/#subcommittee-standard-development-nystagmus" data-outlook-id="f570f891-643e-4552-88f8-87e66a7932e7">27 contributors</a> drawn from a wide range of clinical, technical and geographical backgrounds. While this is a relatively large group, the scale is intentional as its remit extends beyond nystagmus itself. In parallel with drafting guidelines, the group is working to define their scope and test decision-making processes that will underpin future ISCET guidelines. This includes deciding how guidelines will relate to one another. The group is provisionally adopting an ’eye movement systems’ approach, with the aim to define core guidelines for testing systems such as <i>fixation</i>, <i>pursuit</i>, <i>saccades</i> etc, which can be applied across a range of clinical conditions, which remains open to refinement as the guidelines development.</div>

<div>The progress to date reflects strong volunteer engagement and international collaboration across regions and clinical disciplines. It also marks a transition from organisational establishment to substantive standards development, which will directly support clinical practice. In recognition of this shift, the current Organising Committee, who have dedicated their time since ISCET’s inception, has agreed that all committee roles will be open to formal nomination once the first ISCET guideline has been published. Details on the nomination process and timeline will be shared with the membership as we approach publication of the first guideline.</div>

                        <content:encoded><![CDATA[<p><span>Since the second meeting of the nystagmus subcommittee I have been considering how best to reconcile the need for a 'fixation' protocol which stands alone from nystagmus while at the same time providing clear guidance for assessing nystagmus.</span></p>
<p>On the one hand, a 'fixation' protocol needs to be agnostic to the clinical condition (so that it can be used in contexts beyond nystagmus). On the other hand, if we only produce a basic fixation protocol with no guidance on how to apply it in nystagmus, then we aren't really any closer to <span>clinical standardisation (the point of ISCET guidelines is to ensure patients receive a similar standard of care across clinics).</span></p>
<p><span>I would therefore like to propose a 'layered' approach, with the nystagmus subcommittee producing two distinct outputs:</span></p>
<ol>
<li>A 'core' fixation protocol, which does not mention nystagmus, and is generally applicable across a range of clinical contexts. This core protocol could then be reused in other conditions, as necessary.</li>
<li>Nystagmus-specific guidance, which specifies the particular viewing conditions / gaze angles etc over which to apply the fixation protocol. If the group deem it necessary, then other 'core' protocols (e.g. OKN/saccades etc) could also be referenced in this layer.</li>
</ol>
<p>With this approach we would end up with a set of 'core' protocols for each eye movement system (fixation, OKN, saccades, etc), and then a 'higher level' set of disease-specific protocols (nystagmus, Huntington's disease, PSP, etc) which refer to the core protocols.</p>
<p>Would be interested to hear others thoughts on this approach, and I would love to hear suggestions for alternatives or refinements to this. And, so far as is appropriate, we should probably try to replicate the structure of ISCEV protocols, so their guidance will be especially helpful.</p>]]></content:encoded>

<p>Rasha has asked me to point out that both <a href="https://www.conftool.com/ecem2026/" target="_blank" rel="noopener">ECEM</a> and <a href="https://etra.acm.org/2026/index.html" target="_blank" rel="noopener">ETRA</a> are currently seeking abstracts for symposia. If anybody is planning to attend either of these and might be interested in facilitating a presence for ISCET, please let us know. Rasha will keep an eye on this forum post for responses.</p>
